Client Waiver & Service Agreement

Thank you for choosing Hearth & Home Pet Concierge. My goal is to provide calm, attentive, personalized care for your pet and peace of mind for you. This agreement outlines what you can expect from me and what I need from you to keep your pet safe and your home secure.

1. Care & Home Access

By submitting your name below, you authorize Hearth & Home Pet Concierge to enter your home and provide agreed‑upon services, including feeding, walking, enrichment, companionship, and medication administration. All care is delivered with professionalism, discretion, and kindness.
 

2. Health & Emergency Care

If your pet requires medical attention, I may seek veterinary care on your behalf. I will always attempt to contact you first, but your pet’s well-being comes first. You agree to be responsible for all veterinary costs.
 

3. Liability & Safety

By acknowledging this agreement, you understand and agree that:

• I am not liable for illness, injury, or death resulting from pre‑existing conditions, natural causes, or circumstances outside my control.

• You are responsible for any damage your pet causes to property, other animals, or individuals.

• I am not responsible for escapes or incidents caused by faulty equipment, unsecured property, or environmental conditions.

• Behavioral challenges arising from your pet’s established habits or home environment are not my responsibility.
   

4. Behavior & Medical Disclosure

You confirm that you have shared all relevant behavioral and medical information, including reactivity, anxiety, aggression, mobility limitations, or special needs. Undisclosed concerns may affect service availability.
 

5. Medication Administration

If medication is required, you confirm it is prescribed by a licensed veterinarian and that you have provided accurate instructions. I will administer medication as directed, though outcomes cannot be guaranteed.
 

6. Photos & Updates

I love sharing updates and photos during visits. It is your responsibility to let me know if you do NOT want photos of your pet used for marketing.
 

7. Scheduling & Payment

You agree to the posted scheduling, payment, and cancellation policies. Late cancellations may incur fees.
 

8. Acknowledgment

By typing your name below, you acknowledge that you have read, understand, and agree to this service agreement.
